Case Manager/Social Services Case Worker

 

Full Time position with ability to take call.

Work in collaboration with all healthcare team members to facilitate services to ensure care is appropriate, timely, cost effective and achieves desired results for patients throughout the continuum of care.

  • Coordinates communication among healthcare team members including patients, family, physicians and payers.
  • Facilitates interdisciplinary care conferences to provide continuity of care throughout the continuum.
  • Identification of patient care needs for appropriate screening and referrals prior to discharge.
  • Performs patient assessment to include:
    • Clinical status
    • Functional capabilities
    • Patient goals of care
    • Discharge needs
  • Develops action plan for discharge planning including specific tasks and timetable.
  • Identifications of efficient utilization of clinical resources.
  • Tracking, entering and reporting case management variances/avoidable days.

Minimum Education: Associates degree in Nursing or licensed Social worker

Certifications/Licenses: RN license and CPR required. Certified Case Manager or Social Worker preferred.
