RN Care Manager – Remote
Description
RN Care Manager for a remote position. The ideal candidate will possess strong clinical skills, exceptional communication abilities, and the capacity to assess patient needs remotely. Responsibilities include conducting comprehensive assessments, developing care plans, and coordinating with healthcare providers to ensure optimal patient outcomes. Candidates must hold an active RN license and have previous care management experience. Job Responsibilities:
- Conduct comprehensive assessments of patients’ health status and care needs remotely.
- Develop individualized care plans in collaboration with patients, their families, and healthcare providers.
- Coordinate and facilitate communication between patients, caregivers, and healthcare professionals.
- Monitor patients’ progress, assess their response to treatment, and adjust care plans as needed.
- Provide patient education on disease management, medications, and lifestyle modifications.
- Advocate for patients’ needs and ensure access to appropriate healthcare services and resources.
- Utilize telehealth platforms and electronic health records (EHR) systems to document patient interactions and care plans.
- Collaborate with community resources and support services to address patients’ social determinants of health.
- Maintain confidentiality and adhere to HIPAA regulations in all patient interactions and documentation.
