Job Responsibilities:

  • Conduct comprehensive assessments of patients’ health status and care needs through remote consultations.
  • Develop personalized care plans in collaboration with patients, caregivers, and healthcare teams to address individual needs and goals.
  • Coordinate and facilitate communication between healthcare providers, specialists, and community resources to optimize patient care and outcomes.
  • Provide patient education and support on disease management, medication adherence, and lifestyle modifications.
